Form is Disabled?

All my forms currently say "form not found" and that they are disable.

why this happens?

Upon checking your account, I see that you have two forms that were flagged by our anti-Phishing system as suspicious.

You form has field names "username" and "password", this is actually a violation of our Terms, please check the section under "Phishing", our form builder was not designed to create log in forms (which means to capture or ask for username and passwords), this also includes sensitive information like bank details, credit cards, keys and the likes. 

I've edited your forms and deleted the fields that violated our terms for me to re-activate your account. But, please do avoid using or creating similar field names or process, our anti-Phishing system will automatically scan your form and autosuspend your account.
