I've checked your JotForm titled Delivery Message Note and see that the PayPal is already integrated to the form. However, I don't see a submit button on the form. Please add the submit button to the form.
Once the form is submitted, it should be redirected to PayPal to enter the payment information by the client. If you see any issue with the form, please let us know. We will be happy to look into this.
