I have this script::

But now that I've brought all the content over to the domain lboloans.com/loanapplication, the form does not appear. What am I missing?
I've checked the provided URL and noticed that you're using HTTPS to render it and also got a Page Not Found message.
If you want to embed your form using HTTPS protocol then you need to enable SSL submissions in it to do so. Please check this guide: https://www.jotform.com/help/63-How-can-I-receive-SSL-Submissions
Also, note that free accounts are allowed to receive 10 monthly SSL submissions, so if you are planning on receiving more you should consider upgrading your account to a paid plan.
