We continue to have problems with this form in particular. On occasion guests complain that they fill out the form, please submit and get a blank screen. So they naturally think something went wrong and email us. We must go online to jotform to see that they registered. In addition, we do not get an email when this happens which is very frustrating and creates problems on our end. We've had return guests who have complained why we haven't fixed the problem. This has been going on for many months.
- Why do you get a blank screen after the submit button? They should get a THANK YOU notice.
- Why don't we get an email alert for these problem submissions?

I was able to see the Thank You page after submission
and also received the auto email to my email address I provided
According to the sent email logs, emails were also sent successfully to your email account.

If you are still having issue, can try re-embedding the form on the website using instead its Iframe embed code. Make sure to clear first all the form previous embedded script code before re=embedding using iframe.
Check if this makes the difference.
