My form is not working when I embed it to my website

I built a form, installed it in iWeb, once it is published, it will not let anyone put a comment in the area.

Any ideas?  Usually your forms work great!  This is the first time it does not work.  However, you can enter a comment while it is 

However, you can enter a comment while it is in the program, but not after it is published to the web.

Any help?

Please find the following div in your web page (it's the one that contains the background image of your page) and change the value of its z-index to -1 so it would look like this z-index: -1

The image is being placed in front of your form and this is why is not clickable, modifying the z-index value should fix this.
