Information Not Displayed in Email Notification

I created an online questionnaire:

and after I click submit, the contents of the email show this:

how do I get the info from questionnaire to my email?

Hi,

Upon checking your form, it seems that the form field tags in your email content are not correct. They might be the outdated ones when you changed or updated your form.

To add the correct tags, edit the notification in your "Emails" setup in your toolbar, and add the form fields by clicking them at the right side.
