Form Revision system for my jotform does not seem to work correctly

I tried to recover a correct version and it will show during that process but then when I go to edit the form, the wrong version keeps appearing. This is not the first time that changes have not been maintained and is of great concern.

Please advise on how to recover the version we want - I have already tried the recovery process and it appears to recover but does not stay that way.

Now, I would suggest following.

1. follow the steps here: How to View Form Revision History
2. follow the steps here: Clear form cache

This should revert your form to previous state, but also clear the cache so that it is not loading different parts of the jotform from cache.
