2. I used to receive an alert in my email when a new form was submitted. This has stopped and I'd like it to continue.

Your email notification is not properly setup, I see the Sender Name is missing:
Sender Name :Make sure you assign it to a field from your form where the user enters her name (usually the"Name" field -or similar)
In addition to this, you may white-list our domain names, if the issue continues, as we use Amazon SES method for "email@example.com" sender.
