Issues with auto-populate

We are having some issues with our forms.  We have 125 chapters, and each chapter submits various monthly reports.  We've set up autopopulation based on the value of a selection, so if John Doe selects "Arkansas State University, Alpha-Pi Chapter", it will populate "alphapi" into the "Reports Email" box and add "reports@sigmapi.org" to it. This is an example of one of the forms:  http://form.jotformpro.com/form/42864679307972

Some users aren't having that field autopopulate like it should, so we are looking for ways to fix this bug.  I set up a test form where we changed the value of the "Institution" to reflect the full email address, but the Email Alerts do not allow it to be sent to the value, just the Option. http://form.jotformpro.com/form/50416502127949

    Any suggestions?

Can you please tell us if the original issue is reported on the jotform that is being accessed directly, or for one that is embedded on some page?

If it is embedded, could you please give us the link to the page to test it out?

Now I am not sure what you mean by the test jotforms option on institution field?

Do you mean that the email notification / autoresponder is not picking up the option name or the option value?

Based on your email setup, I see that the Sender name is set to undefined, which is never a good option.

I would suggest taking a look at these 2 guides in regards to this:
