Form Layout Issues

I am having a very difficult time returning my from to the default settings.  I mistaking wanted to see what a theme would look like and now can't get things to go back to their original look.  The form is the Contact Page form on my account.  Please help!  The page is not yet life but in my jotform editor it looks terrible.
