I apologize for any inconvenience this might have caused but it appears that your account was suspended automatically because your form was originally made in Arabic language.
To avoid a recurrence of this I would recommend building the form in English language and use the Language Translator tool to traverse between the your desired languages.
Forms that are made from French, Arabic and other similar languages will be automatically suspended by our Phishing Detector because it was not updated to scan these languages. Our tool is properly equipped to suspend accounts that has forms that is being detected of phishing activities such as stealing credit card information, social security number as well as log in credentials among others.
As suggested above, to avoid another automatic suspension, it is recommend that you follow this guide to set up a multi-language form. Ensure the form is placed in English language by default. I have reinstated your account so you should be fine. Do let us know if further assistance is needed.

I checked your account and your form and everything seem to be in order. Can you provide more details on your current issue?
Our Phishing Detection System is not yet optimized to scam French or Arab among other languages so most times when forms are written in these languages they score a high percentage by our system and an Automatic Suspension is placed on the account. If the form was originally made in English and our Language Translation option was used to convert the text into Arabic you would be just fine.
I would recommend that you clone the existing form that you have and change the labels to English. You should then use the Language Translator to map the Arabic statements to the English statements in the form. I will translate your form into English and set up the Arabic translation and provide the link for it so you can clone via URL and make further changes. I will update you on this soon.
