Form Disabled?

Unfortunately, I'm unable to fully understand your concern or question. But I do see that your form is currently not available?

Upon checking it, I noticed that it has a password field, please do remove this part of the form, we do not allow forms that ask for sensitive information, these includes log in credentials (username or password), bank information, credit card details, and the likes, this is a violation of our Terms, please read the "Phishing" section of our Terms page..

Another possible reason is that your form is in Arabic language, you might want to check my reply on this thread: Form Disabled: Form in Arabic Language 

I've removed the field asking for the password and re-activated your account, but please do avoid creating forms that violates our Terms.
