Forms were Disabled

My two forms has been disabled? why? please help me

Hi,

Upon checking the form you have, your forms were disabled because the account was autosuspended. This is because our Anti-Phishing system have detected that you have fields asking for "username" and "password". Please do take note that we do not allow forms that ask for sensitive information, these includes log in details such as username and password, bank information, credit card details and the likes. ok but my second form is not asking username and pasword please look at my forms and unblock.

Your forms are available now. Your account was suspended because of your other form that contained fields asking for "username" and "password", when your account gets suspended, all your  "good" and "suspicious" forms are suspended in cascade.
