Upon checking the form you have, your forms were disabled because the account was autosuspended. This is because our Anti-Phishing system have detected that you have fields asking for "username" and "password". Please do take note that we do not allow forms that ask for sensitive information, these includes log in details such as username and password, bank information, credit card details and the likes. Please do check our "Terms" page under Phishing section.
For now, I've edited your form and removed the fields that made the violations, please do avoid creating similar fields or forms, your account will automatically be suspended then.

Your forms are available now. Your account was suspended because of your other form that contained fields asking for "username" and "password", when your account gets suspended, all your "good" and "suspicious" forms are suspended in cascade.
