I'm a volunteer at Dare To Dream Youth Ranch. We are a not-for-profit organization, see...

We rescue Horse in the twilight of their years or career as a Racing or Working Horse.

We offer Equine Therapy (care, grooming and riding) to children with emotional and/or physical disabilities.

We need these children and their guardians to sign assumption of risk forms and we also need a single place to hold all their data, contact, medical etc…  We need to be able to show our  State Government a "paper trail"  of what we do, who we serve and when. An added benefit would be to use this collected data to market too for fund raising so that we can continue to serve our local community.

I'm int he process of creating Google forms and discovered you service, it looks to be a more robust way to mange the data. It also seem easy enough for me to rain other volunteers.
