I was charged twice for premium subscription.

    It seems your account "peterCuocolo" is actually a free account. Upon checking, I found that you have another account "pcuocolo" which is upgraded to premium monthly subscription. 

    You seems to have two active subscriptions:

    INT150126-5417-40506S upgraded on January 26, 2015.

    INT141127-6617-34641S upgraded on November 27, 2014.
