Form Disabled

    Hi,

     

    I need this to provide logins for my associates at an upcoming convention - please help!

    Hi,

    The form http://www.jotform.us/form/50424531396150 was detected by our anti-Phishing system and marked it as a phishing form, it will also automatically suspend your account, this is because it is asking username and passwords, this is a violation of our Terms, see the section under "Phishing". We do not allow users to create forms that asks for sensitive information, these includes log in credentials, passwords, bank information, card details, and the likes. 

    What I did right now is I edited your form and deleted the fields that has violated our terms, I've reactivated your account so that you can continue using it, but please refrain from adding or using field names that ask sensitive information, our system will continue to detect those kind of forms and autosuspend your account.

    Hi there right now it isnt showing the account has been reinstates its still showing that the form has been disabled :( is there anything else i need to do on my end? thanks!

    Hi,

    Apologies for that, I see that the system is also detecting the field name "Log In" as part of a phishing form. That field is the one with the drop down option "Investor" and "Client". I've re-activated your account, see that form and delete the field or rename it to avoid being autosuspended again.
