    Allow Multiple Choices To be Selected?

    How can I allow multiple choices to be selected in a form (i.e. select all that apply)? How did you hear about us?  (Please check all that apply)

    Instead of using the radio button, you can use the check box field to allow multiple choices to be selected, you can also add a "other" field there.
