My former GM Randy signed us up for Jotform, we use it for rresumes/applications through our website. Since terminating him, our resumes have stopped coming in. I know the payment for your services came off of my credit card. I urgently need to address this issue. I huess I need our log in info.

May I know the form URL or the website where the form is? I would suggest that you contact the person to have the log in details of the account and update it to your own. The log in details like passwords are encrypted in our system and we cannot have or view it in our end, and for security reasons, only the owner of the account can have access to it.
Or you could just remove the form embedded in your website then create your own account and clone that form, here's a guide on how to clone a form: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL.
