I did check your web page where you have embedded your form. You seems to have embedded two forms there. Please check the screenshot below:
I did check your form's submission page and It seems your second form is not submitted at all. If the first submit button of your web page is clicked, user will miss filling up the second form. I believe that is the case here.
The second form is configured correctly and if you input fields value and submit form, you should receive the submission correctly. I did test your second form and it seems to work as expected and you should have received the submissions correctly.
One issue I found that you have embedded both the form with script embed code and it could cause the conflict issue. When I submitted the second form, I was redirected to a captcha page.
