Embed two forms on a webpage

I still can't get my new client form to work correctly http://www.breakingthelinks.com/forms I know you said something about two forms but I have tried everything to fix it.

I see that a form is embedded on the weblink that you have provided here. Do you want to embed two forms on the same page? If so, you can try embedding using iframe embed option for both forms on your webpage. You can get the iframe embed code for the forms by following the instructions provided in the guide below :
