I still can't get my new client form to work correctly http://www.breakingthelinks.com/forms I know you said something about two forms but I have tried everything to fix it.

If so, you can try embedding using iframe embed option for both forms on your webpage. You can get the iframe embed code for the forms by following the instructions provided in the guide below :
