How to create a form letter?

i would like to create a letter with letterhead that has set text and ability to add additional text.

then email a PDF to myself and to the email of the recipient.

I would like to confirm if I understood you correctly.

You would like to set up a jotform using our JotForm Builder that would have certain text and values shown, but would also allow you to:

1. have extra text fields to add data to and
2. download the PDF of a form with the form questions and header as it is shown above, together with the entered data?
