Your account was automatically disabled by our anti-phishing filter due to credit card fields on the 'Classic Travel Connection' form. We do not allow collecting credit card details using jotforms.
If you would like to accept online payments, please consider using payment processing services integration. The major payment integrations (PayPal, Stripe and Authorize.Net) also support Authorization only payments.
Your account has been re-activated now, but you will need to remove/redesign mentioned form. Otherwise, the account might be suspended again due to the credit card fields.
