Your form allows images but I think only inbetween individual form fields.
I have adequate spacing to insert an image on the other side (see example)

This may be possible; but my first thought is:
Is there no way you could embed the image into your webpage itself?
you could try putting
<img src="/pathto/myimage.jpg" style="float:left;margin:0 5px 0 0;" /> right after the code you used to embed your JotForm.
It justs seems a more natural way to do it.
I am happy enough to see if i can get your image positioned as you want into the form itself, its just going to be that bit more complicated.

Sorry to butt in, but I noticed that you are using an unregistered guest account, which is limited to only 20 submissions per month and relies on browser cookies for identification. To prevent further problems down the road, please register a free JotForm account using the same e-mail address you have used as your form's notification recipient.
This should allow you to access your forms and data from any device, location, or browser.
