We are constantly lose our integration with our Google Drive. It happens to all our forms at different times. When we reapply the integration everything works fine but sometime later (a month, two months..) it happens again. It doesn't seem to happen for any particular reason, it just happens.

I have not seen reports about Google Drive integration not working consistently, have you tried clearing browsers cache? Also try clearing your forms cache: http://www.jotform.com/help/229-How-to-Clear-your-Form-Cache, this may help fixing any corruption on the form.
