Everytime I submit my form it create 2 entries instead of 1.
I have gone through and through and cannot see why this is doing this, I have only one button on the form for submitting data, this is also reflected when it sends confirmation email and into the google sheet that is linked.
My form causing the issue is
It has got to be something so silly that I cannot see ?
Any help would be great as this is hammering my submissions after previous issues with google integration not working and I end up finding the problem myself that it is the Unique ID that was doing it.

I've tried submitting your JotForm and see that one entry is posted through the submission. This may be due to the removal of the widget as you suspect. I've tried cloning your JotForm to my account and submit it. I was not able to replicate the issue even after adding unique id widget on my cloned form.
Also, I notice that you are now making some changes to the form. Could you try adding the unique ID widget now and let us know if the issue arises again? This will help us in investigating the issue and provide you with necessary assistance.
