I see that you are subscribed to premium monthly service. Since JotForm is a monthly subscription service, it automatically renews the following month upon the due date. However, a renewal reminder will be sent to the registered email address. If you want to cancel the service before June 2015, you can request for a cancellation/downgrade of subscription right after renewing it for the last time i.e., after May 2015 renewal.
You can switch to free plan anytime from the pricing page while you are logged into your JotForm account. The account will be holding premium status until the next billing date and then will be converted to Free account.
