Import Excel data And generate Unique ID and QR Code

    Dear, have a very good afternoon.

    I just created the following form: https://form.jotform.com/231460822374655

    This form has the text fields but also has the field: Unique ID and dynamic QR that receives data from the QR calculator

    I need to import an Excel database where I only have: First Name, Last Name and Telephone Number

    Only these 3 fields, but when I import them to the form I need to assign a Unique ID and generate the QR code

    Can you help me with this thank you so much.

    Thanks for reaching out to Jotform Support. Unfortunately, I fully didn't understand your concern. Can you explain a bit more about your requirements so we can help you on this further? It will be great if share an example explaining what exactly you want. Are you looking to get data from an excel sheet to the form and then submit a form based on those prefilled data? If yes, you can use Spreadsheet to form Widget. This will allow you to fetch the data from the excel sheet to the form and then you can submit the form along with some other data like QR code, unique ID etc. to the Submission table in Jotform.

    Once we hear back from you, we'll be able to help you with this.

    Thank you for your answer, I was able to move forward with something but I have some questions.

    I have an excel spreadsheet loaded in the "Spreadsheet to Form" widget

    Problem 1: I can import the data, but not the NAME field

    Problem 2: Once I can import the complete data from a form, I need the button to finish and save the data, so that it is available in the table and save the QR code.

    But the finish button does not appear.

    Hi dnannini,

    Thanks for getting back to us. Unfortunately, I didn't fully understand the issue about the import name field. Are you not able to use name field on the spreadsheet used for the Spreadsheet to Form widget? Can you explain a bit more about the issue so we can look into this further?

    As for the button not visible, are you referring to the submit button on the form. If you are not able to see the submit button on the form. If you could please share a screen recording explaining the issue that will be great.

    Once we have a better idea of what’s going on, we’ll be able to come up with a solution.

    Hello Raúl, thanks for your answer.

    You can test the error by entering the form and enter a Unique ID number, for example use 5 and you will see that it does not show the data in the FullName fields

    Hi dnannini,

    Thanks for getting back to us. ​​I tested your form and had the same issue. Please ensure that the form field labels and spreadsheet column names match. Can you share the file with us via Google Drive and provide us with the public link so we can check it out? 

    Regarding the send button you're referring to. Is it the submit button? If this is the case, please try removing the old submit button and replacing it with a new one.
