Does JotForm have a blank template so I can create my own forms from scratch? I'm not seeing where this is.

I was unable to find one so I created one for my own use.  I named it *Template so it shows up at the top of my list of forms.  Hope this helps.

To create a blank form, go to http://www.jotform.com/myforms/ the click on "Create Form":

This will allow you to create a blank form and add the fields you would like.
