Why is my account suspended?

suspected of phishing and I am not. 

I started working on a form as I thought I was logged in, but was not.  Not I am concerned I will lose the form.  I also need the form linked to my account as the form is active on my sight.

I've deleted the culprit fields of your form and reactivated your account, but be aware that you must refrain yourself to do this again or your account will be suspended.

Forgot to mention that regarding the other form you're working on, we would need to know the ID of it or the email address that you used to create it in order to be able to associate it with your account.
