That is due to script conflict. The required validation and the page break function breaks due to the conflicting scripts between the form scripts and your website scripts. Sorry, after checking your website source I think it's not possibly due to script conflict. I think it's due to the fact that non-https resources like the form scripts are blocked and wasn't executed on the browser since your website runs on SSL (https).
Here's a solution. Find all the http links on that page source and replace them with https, that might fixed the problem.
