Just wanted to report a problem I am having with the form.
I am not receiving submissions again. This happens a few times a year.

I believe that you are referring to the JotForm titled "Moving Help Center 2015 (Main)". I see that the emails are being sent from our end without any issue. It seems that you have tested email setup by clicking on Test Email.
