Forms disabled

I was designing forms and suddenly my forms begin not responding. It says they are disabled.

Your account was suspended and the forms were disabled because our team has detected that one or more of your form has violated our Terms. The form named "Clone of Chaser" has the fields "username" and "password", please see the Phishing section of our Terms page, we do not allow users to create forms that ask for sensitive information, these includes log in credentials, passwords, bank details, credit card information and the likes.

I've deleted the form fields or better you could delete the form right now, I have reactivated your account so that you can make proper adjustments. But please do avoid creating similar forms that has violations, we will continue to suspend and disable them.
