How can I print the submission to look exactly the same as the form template?

 I want the form to look exactly the way it did when I sent it, but with the answers in the associated text boxes.  Therefore, I can print out the form and put it in a personnel file.

Unfortunately, we don't have a direct feature that you can use to accomplish this. A workaround would be to add a Print button in your form:

Then add a notification email to your form: https://www.jotform.com/help/25-Setting-Up-Email-Notifications. 

In the notification email body, add the edit link:

So, after you receive the submission through email, click the edit link and print it using the print button in the form.
