Combining checkbox and text-box fields in the same line

I have attached a picture  of an example of what I am trying to do.

We can achieve a similar result by using the spread into columns option on the checkbox/radio button fields:

Then you need to shrink the field:

Maybe you need to increase the form's width so you can drag the text-box field to the same line:
