Why is my form working on one page and not the other?

I have a JotForm in the widget area of my Wordpress site.  On the home page it works correctly but on other pages it doesn't work correctly.

The pages it doesn't work on are parent pages, why is this making a difference how the form is showing up?

I see that there are two JotForms are embedded on the same page www.empireunderwriters.com/workers-comp/ that I believe a script conflict between each other. Could you try embedding one of the forms using iframe method and see if the issue is resolved? You can get the iframe code for your JotForm by following the instructions provided in the guide below :
