I looked through widgets but did not see anything like this
I am looking for a form that can only be activated by entering a known code into the first field of the form. I would supply this code to the person that I wanted to fill in the form.
Once the code has been entered and the form submitted that code could no longer be used. There would be a list of valid codes maintained by me.
Is there a way to do something like that?

There is no specific widget that can do this
BUT you can use -How-to-set-Form-Limits-Based-on-a-Unique-Question and condition logic of the form.

In my demo form I setup 2 code test123 and test12345 .
I already used test123 so you cannot use it anymore
try testing the form using test12345.

Btw, you can also clone my test form in your Form Builder so that you can check how I did it.
