Added the option that would allow a customer to attached a document when submitting a form. However, the attached document fails to send to the appropriate recipient address as an attachment.

Please try re-creating the Notification E-mail of your form. You can use the Add New Email button to do it.
the new Notification template should include then the upload link to the Upload submission of the form.
