How do I create the Bar Link for the form?

    Okay, so I used a template and this was a bad idea as all the e-mail forms come in as Salon Employment Application -- not my field names so it is totally confusing.

     

    Problem is I used this template to generate several forms so now I get all my forms sent to my email like this.  I can go onto JotForm -- THANK GOD -- and see the forms there.

    However, I would like to be able to set the form links into my website like this template has them -- with one Bar that has the form name -- once the person clicks on the bar the form opens up so they can fill it in.

     

    I don't know how to do this and can't seem to figure it out.  Can someone help me?

     

    Also, Can someone help me get the forms sent to my e-mail like the forms that are stored in JotForms?
