    I created a form as PDF but only some fields work

    I have created a form as it as pdf and when I tested it all fields worked except the ph#, email & country how can this be corrected

    To make sure I understood you correctly, you created a form as PDF and the phone, email and country fields are not working:

    PDF forms are more directed to printed versions. So from my tests only text-box, text area, full name, address (except the country) allow digital editing.

    If you wish to digitally fill in your PDF form it is necessary to use one of the PDF editors available. Some are free to use.
