My account suspended due to

    Hi! Our website is http://dentalmastersindiagnostics.com. We are a dental diagnostic services and currently upgrading our site. I love jotform and we are not "phishing" anything. We are building a service wherein our clients can order their services online that will require a membership. I'm currently testing it but jotform suspended our account. We are about to launch and introduce this site on November 15 for the PDA Convention. Kindly let me know what to do asap. We are legitimate company and I'm even requesting the proprietor of our business to get your services but you suspensed my account. I' m using my personal account because I'm still testing it. Kindly respond to my concern asap.

    Hi! That's quick. I login again and it seems to be working. Thank you. Hope I won't get a problem in the future. Let me know what your site need to have an info about us, so, this will not happen again.

    Sorry about this.

    Your account was auto suspended because of your form that looks like a login form.  JotForm cannot process logins so our anti-phishing form scanner normally triggers a suspension for such looking forms.

    I have now reactivated your account.  You can now log back in.  

    For your membership with sign in needs, you will need to use another application such as Profile Manager Basic, Profile Manager Premium or Memberkit.  These applications were developed by the same creators of JotForm.  You can visit this page for comparison of these three applications.
