You can configure your PDF to include some fields or all of them, depending on your preferences. This is easily done over the edit options:
Once you click on it you will see checkmarks to decide which fields you want and which fields you do not want to be shown.
Now to include the Headers and Text from the jotform as well, I would recommend taking a look at this guide:
The only thing that I would recommend that is not shown in that guide is to make sure that Auto Hide Empty Fields is unchecked if you want the unanswered questions to be shown in the PDF as well.
