Your form is available at https://secure.jotform.com/form/12484058174
I do notice an issue with the https://www.jotform.com domain on your form however. Its only affecting your form, its not site wide.
One thing I did notice, your images arent stored in a secure location, so web browsers will report that the form isnt on a completely secure connection. Its probably worth changing that.
On your embedded images, can you change their location to a secure one & resave your form? (just put a https:// instead of http://
Can you just change those & let us know if that helps at all.

I changed the link to https://secure.jotform.com/form/12484058174 as instructed and it appears to be working now.
Was that simple change all that needs to be done to make the forms secure?
Thanking you in advance, Richard

https://secure.jotform.com & https://www.jotform.com are effectively mirrors. Both style addresses are valid for secure transmissions. Very occasionally we see synchronisation issues. It may just take https://www.jotform.com/form/12484058174 a few minutes to 'catch up', however it is usually instant.
& yes, only the change of address to that https://secure.jotform.com/form/xxxxxx format is all that is required to have any form made 'secure'
I have checked your form in a few browsers now, all images fine & all content is fine. All reporting your form on a completely secure connection.
Hope that helps.
