This may be out of scope for JotForm, but I am looking for a way to administrate simple employee evaluation form for a small group (20).
Basic needs are as follows:
Employee enters some personal information and then (self appraisal) answers four radio button questions, and each would allow for additional comments.Employee can save and come back to the form but at a defined cutoff it would become uneditable to them.At that time, up to two supervisors can go in to the form and review the employee answers and also the supervisor can enter their own comments/responses on a per question basis.When the supervisors are done, data from the entire form would need to be printed on hard copy.
Am I barking up the wrong tree?

This is indeed possible with Jotform. We have multiple widgets and apps that could facilitate your need. You can use fields from our Quick tools tab to populate for form with fields such as Full name, email address, address among others.
You can use regular radio buttons to provide ranking or you can also explore the use widgets such as Flat Radio Buttons.
Conditional logic statements can be used to show/hide text area fields for the additional comments to be entered.
