    How do I create a field for 'please specify' when an opion is clicked?

    e.g I'm doing a question about what the respondent's pension includes. Options such as:

     

    Company Car
    Housing allowance
    Private Medical Insurance
    Death in Service
    Canteen
    Season Ticket Loan
    School Fees
    Cash Equivalent (please specify)

     

    I need there to be a box that appears when 'cash equivalent' is ticked that asks the user to specify the amount.

     

    Please can you help?

    What you are looking can be easily accomplished by using conditions in your form
