The JotForm application previously named Applidok has been renamed to Fudok, but for whatever reason it is disabled.

Though I am not sure what the applidok app did, it does appear as though it has been removed:
I was able to find the Fudok application and it does appear to be enabled:
It may have been altered by our dev's. If there is something we can help you with in regards to this application, let us know and we will be happy to see what we can do.
