How can I import my Adobe Form Central forms?

You can import your Adobe Form Central forms by submitting a request here:
http://www.jotform.com/formscentral/ 

You have the option of import your forms only or you can import both your forms and your data. Data would include previous submissions information, file upload among others made via your form while it was on Form Central. If you follow the link provide all the necessary information will be provided to you.
