Asking for password in a form

My account was suspended because on a form I asked for a username and password, I explained that the form is for a member to close their account. I need to make sure it is them closing their account and not someone other than the owner of that profile. The information is information that I already have, But the owner of that profile and I, should be the only people with that information. That is why I asked for the password. To Verify it is them.

But my question now is it ok to do this; I don’t want to break any rules. But Can I rebuild that form or not. In the past I have had people close accounts that they did not own, and it can make problems. Please let me know if I can do that now.

My account is back now, But I deleted the form in question, But I don't think I needed to, They had reinstated my account before I deleted the form, Or I could not have logged on to delete the form

You can read more about what you can and can't do with your forms here: http://www.jotform.com/terms

It's against our terms to ask for sensitive information such as passwords, so the answer is no, you cannot rebuild that form.

Let us know if you have further questions.
    Thanks.
