I set up JotForm to receive files and tested by sending a photo. But all that showed up is a PDF with the submission date. Do I need to modify any settings?

First of all, I recommend creating an account also before proceeding. You can use this link http://www.jotform.com/signup to signup using your email account.
As to the inquiry...
I checked the form on the website, I noted that it seems to be not the updated version of the form.
Try clearing first all of the embedded code of the form, and then re-embed using its iframe embed code instead. Then test/check again afterwards.
