I would suggest you to please explain the problem you are having with your form a bit more in detail and we will surely help you. You may also like to share a screenshot of the problem as well. Thanks for yr help, yes the form works, but I don't receive the notification to my mail address. I have not received yr test but I can find it on my jotform account.

I'm sorry for the trouble caused to you.
I did check your form's notification email alert and found that you have not yet selected "Sender Name". Please check the screenshot below:
Please be noted that if you do not provide sender name, the incoming server will most likely treat it as spam or simply ignore it. That could be the reason why you are not receiving the submission emails from your form.
I would suggest you to select "Sender name" and see if that solves your problem.
Do get back to us if the issue persists.
