Need Help Creating A Form Like This

Hi There

Ive already got the upload file sorted out, but the rest i need help with. Do you some of you know what the others are, like example what product to select ( i know its a drop down, but it must show images ect). Also there is a option to add another order, can this be done on jotform were i press a button, it make a new order underneath the first order

The "Add another item" feature can be achieved with some conditional logic, where you add a form collapse and then duplicate the fields:

You also add a check box field "add another item" that when is filled will show the duplicate fields.
